Facilities Usage Request Form

Please complete and submit this form at least 3 weeks in advance of the event, when possible. Requests are booked on a first-come, first-serve basis. Preference will be given to internal church-sanctioned events.

Pricing is for a 2hr Minimum per day. (i.e. 30min before + 1hr Event + 30min after)

If Food is being served during this event you will need to provide plastic tablecloths to avoid additional costs

Should you require the use of the in-house sound system, media or projection equipment, video recording, or live-streaming services, at least one technician from the Fountain must be present. A fee of $100 per tech is applicable for the first two hours, with an additional charge of $25 per hour for each technician beyond the initial two hours.
